What’s in this T-shirt order form?
Ask for a name and an email, then the style, size, color and how many. Sizes run from youth small to adult 3XL in one dropdown.
People who want a name printed on the back are asked what it should say, up to 12 letters. Everyone chooses pickup or shipping, and only shipping orders ask for an address.
Edit the wording, add or remove questions, and change the look in the editor.
