Inside the template · 4 questions
What’s in this event registration form template?
Guests give their name and the email address where you should send event details. Then they answer “Will you be joining us?” with yes, maybe or no.
A final, optional question asks whether you can do anything to help them take part, such as meeting an access need. The first three answers are required, so every RSVP is complete.
Edit the wording, add or remove questions, and change the look in the editor.

Events this registration form suits
It suits small, friendly events: a workshop, a community meetup, a launch, a webinar or a get-together. One link replaces a long thread of replies.
The maybe option keeps undecided guests in touch, and the last question gives people a simple way to tell you what they need. Each RSVP lands in your trulyform inbox.
Ways to adapt the registration questions
- Help maybes decide: with logic, show a question only to people who choose “Maybe, please send me more information”, such as what would help them make up their mind.
- Offer sessions or dates: add a multiple choice question that lists the sessions people can join.
- Plan the food: if you are serving a meal, add a question about dietary needs.
Make these changes in the editor. The four starter questions are listed above.

A few things you might want to know.
What should an event registration form include?
A name, an email address and a clear RSVP are enough for most small events. Add questions about sessions, dietary needs or access only when the answers change how you plan.
Is this event registration form template free?
Yes. You can adjust it without an account. Create a free trulyform account to publish it and collect RSVPs within the Free plan’s monthly allowance.
How do I add this form to my website?
When you publish, a “Share your form” window opens. Copy the public link for an invitation email or a “Register” button, or paste the code under “Embed on your website” into an HTML or embed block on your event page. You can reopen the window from the form’s settings.
Does trulyform email event details or reminders to guests?
No. trulyform collects RSVPs but does not email your guests. The ending message tells them to watch their inbox, so send the details yourself, or change that message to say how you will be in touch.
Can I close registration when I have enough people?
Yes. Close the form from its settings at any time, and reopen it if plans change. Settings also include a response limit. It counts every completed response, including maybes and noes, so it is not a count of confirmed guests.
Can I keep the guest list in a spreadsheet?
Yes. Connect Google Sheets to add each completed RSVP as a row, or export a CSV file at any time. Airtable, Notion and webhooks are also included on every plan.
