What’s in this NPS survey template?
The first question is the classic one: how likely is someone to recommend you? They answer from 0 to 10, between “Not at all likely” and “Extremely likely”. This answer is required.
The second question asks for the main reason behind the score. It is optional, so the survey takes seconds when someone has nothing to add.

How Net Promoter Score works
Net Promoter Score sorts people by their answer. Scores of 9 or 10 are promoters, 7 or 8 are passives, and 0 to 6 are detractors. Your score is the percentage of promoters minus the percentage of detractors, so it can range from −100 to 100.

Ways to adapt the NPS questions
- Ask detractors something different: with logic, show a question such as “What would have made it better?” only to people who score 6 or lower.
- Name what you are asking about: replace “us” with your product, a recent order or a particular service.
- Know where each answer came from: add a hidden field such as plan or source in the form settings, then include its value in the link you share.

A few things you might want to know.
What is an NPS survey?
An NPS survey asks how likely someone is to recommend you, on a scale from 0 to 10, usually followed by a question about why. The answers let you work out your Net Promoter Score and see the reasons behind it.
Is this NPS survey template free?
Yes. You can change it without an account. Sign up for a free trulyform account to publish the survey and collect responses within the Free plan’s monthly allowance.
Does trulyform calculate my NPS?
No. Each score and reason is kept in your response inbox. To find your NPS, export a CSV file or connect Google Sheets to add each completed response as a row, then count promoters and detractors.
How do I add this form to my website?
When you publish, a “Share your form” window opens. Copy the public link for an email after a purchase, or paste the code under “Embed on your website” into an HTML or embed block on a page. You can reopen the window from the form’s settings.
How often should I send an NPS survey?
Often enough to notice change, not so often that people tire of it. You might ask after a key moment, such as a first month or a finished project, or every few months for ongoing customers.
Can my team see new scores quickly?
Connect Slack to post each completed response to a channel, with a link to the full response. Your organization’s owner also gets an email for the first completed response, then grouped summaries at most once an hour.
