What’s in this customer feedback form template?

It opens with a required satisfaction rating from 1 to 5, running from “Very dissatisfied” to “Very satisfied”. Two open questions follow: what worked well, and what could be better.

The last question asks for an email address, only if the customer would like to hear back. Everything after the rating is optional, so a busy customer can answer in moments.

What customer feedback tells a small business

A rating shows how someone felt. The open questions show why, in their own words, and that is usually where the useful ideas are.

Send the form after an order, an appointment or the end of a project. Responses collect in your trulyform inbox, and the optional email lets you thank someone or put a problem right in person.

Ways to adapt the feedback questions

  • Follow up on low ratings: with logic, show an extra question only when someone rates their experience 2 or lower.
  • Ask about one part of the experience: add a single choice question about delivery, support or the product itself, so you can compare answers by topic.
  • Change the scale: set the rating from 1 to 10 instead, or rename the end labels to suit your question.

What should a customer feedback form ask?

Start with one rating question so answers are easy to compare, then ask what went well and what could be better. Keep it short, and offer an optional way to follow up rather than requiring contact details.

Is this customer feedback form template free?

How do I add this form to my website?

When you publish, a “Share your form” window opens. Copy the public link for a follow-up email or a “Leave feedback” button, or paste the code under “Embed on your website” into an HTML or embed block on a page. You can reopen the window from the form’s settings.

Can customers leave feedback without giving an email?

Yes. Only the rating is required. The follow-up email is optional, and you can remove that question entirely if you prefer.

How is this different from an NPS survey?

An NPS survey asks one thing: how likely someone is to recommend you. This form asks how satisfied they were and invites open comments, which suits feedback on a specific order, visit or project.
