What’s in this contact form template?

Ask for a name and the email address where your reply should go. An optional choice sorts each message by topic, from your services to something else.

The last question is where people write what they want to ask. Three of the four questions are required, so every message arrives with a way to reply.

Instead of an email link

When a contact form works better than an email address

An email link needs a mail app on the visitor’s device. A contact form works right on your page and asks for what you need, so each message arrives ready to answer.

Put it on your contact page, in your footer or behind a “Get in touch” button. The topic shows at a glance whether a message is about new work or an existing project. Completed messages wait in your trulyform inbox.

Ways to adapt the contact questions

  • Use your own topics: replace the four choices with the reasons people really write to you, such as orders, bookings or press.
  • Offer a phone option: add a phone question and leave it optional for people who would rather talk than type.
  • Ask which project: with logic, show an extra question only when someone chooses “An existing project”, so you know which one they mean.

    Change the welcome message, the topic choices and the thank-you screen. Pick colors and fonts that match your site, then preview the form.

    Choose Publish and sign up with Google or email. Your changes are kept, and you finish publishing the same form.

A few things you might want to know.

What should a contact form ask?

As little as it can. A name, a reply address and space for the message cover most needs. A topic choice helps you sort messages without more typing.

Is this contact form template free?

Yes. Open it and make changes without an account. A free trulyform account lets you publish the form and collect responses within the Free plan’s monthly allowance.

How do I add this form to my website?

When you publish, a “Share your form” window opens. Copy the public link for a “Contact us” button or a menu item, or paste the code under “Embed on your website” into an HTML or embed block on your contact page. You can reopen the window from the form’s settings.

Will I get an email when someone sends a message?

Your organization’s owner gets an email as soon as the first completed response arrives. Later messages are grouped into a summary email, sent at most once an hour. Completed messages stay in your response inbox, ready to export as a CSV file.

How do I reply to a message?

Reply from your own email, using the address given in “Where should we send our reply?”. That question is required, so every completed message has one.

Can messages go to Slack so my team sees them?

Yes. Slack can post each completed response to a channel you choose, with a link to the full response. Google Sheets, HubSpot, Notion, Airtable and webhooks are also included on every plan.
