Inside the template · 5 questions

What’s in this lead capture form template?

Ask for a name and an email address, then, optionally, the name of their business. One required choice asks what would help most, so you know how to reply.

A final open question leaves room for anything else. Only three answers are required, so the form is quick to finish.

Edit the wording, add or remove questions, and change the look in the editor.

Before your first reply

Who a lead capture form is for

A lead capture form gives someone curious about your work a clear next step. They tell you who they are and what they need, so your first reply can be useful.

It suits freelancers, consultants, studios and local service businesses. Add it to your website, social profiles or newsletter, and read each completed response in your trulyform inbox.

Ways to adapt the starter questions

  • Name your real offers: rename the three choices after what you provide, such as a consultation, a quote or a workshop.
  • Add a follow-up with logic: show an extra question only to people who choose “Talking through a project”, such as when they hope to start.
  • Ask how they found you: add a single choice question that lists the places people usually hear about you.

These are ideas to try in the editor. The five starter questions are listed above.

A few things you might want to know.

What is a lead capture form?

It is a short form that turns a visitor’s interest into a contact you can follow up with: a name, a way to reach them and enough context for a helpful first reply.

Is this lead capture form template free?

Yes. You can open and edit the template without an account. Create a free trulyform account to publish it and collect responses within the Free plan’s monthly allowance.

How do I add this form to my website?

When you publish, a “Share your form” window opens. Copy the public link for a button, a menu item or your email signature, or paste the code under “Embed on your website” into an HTML or embed block on your page. You can reopen the window from the form’s settings.

How will I know when a new lead comes in?

Your organization’s owner gets an email as soon as the first completed response arrives. After that, new completed responses are grouped into a summary email, sent at most once an hour. Completed responses are kept in your response inbox, ready to export as a CSV file.

Can new leads go straight to my CRM or spreadsheet?

Yes. Connect HubSpot to create or update a contact for each completed response, matched by email. Google Sheets, Slack, Notion, Airtable and webhooks are also included on every plan.

What questions should a lead capture form ask?

Keep it short. A name, an email address and one question about what the person needs are often enough. Add questions about budget, timing or location only when the answer changes how you respond.
