Inside the template · 6 questions

What’s in this attendance form template?

People give their full name and email address, then the class, meeting or training they are here for, and today’s date. A single choice records whether they joined in person or online.

The form ends with a statement each person confirms: “I confirm that I attended this session.” All six answers are required, so every sign-in arrives complete.

Edit the wording, add or remove questions, and change the look in the editor.

Instead of a paper sign-in sheet

Where an online attendance sheet helps

A paper sheet has to travel around the room, the handwriting is not always clear, and someone types it all up afterwards. With a link, everyone signs in from their own phone, in the room or on a video call.

Show the link on a slide, post it in the meeting chat or a group message, or turn it into a QR code with any QR tool and leave it by the door. Each sign-in lands in your trulyform inbox, with the session and the date beside the name.

Ways to adapt the attendance questions

  • List your sessions: replace the open session question with a single choice of your classes or groups, so each one is spelled the same way every time.
  • Check in with online attendees: with logic, show a question only to people who choose “Online”, such as whether the sound and video worked for them.
  • Ask what your records need: add a question for a student number, a department or a company name, if your school, HR team or training report asks for one.

Make these changes in the editor. The six starter questions are listed above.

What should an attendance form include?

A full name, an email address, the session and the date cover most classes and meetings. The email tells apart two people with the same name and lets you reach them later, since trulyform does not email attendees. Add a student number or a department only when your records need it.

Is this attendance form template free?

Yes. You can adjust it without an account. Create a free trulyform account to publish it and collect sign-ins within the Free plan’s monthly allowance.

Can I use one attendance form for every session?

Yes. That is what the session and date questions are for. Share the same link each time, then sort the sign-ins by session and date in Google Sheets. If you would rather keep each class apart, make a separate form for each one.

How do I get the attendance list into a spreadsheet?

Connect Google Sheets to add each completed sign-in as a row, or export a CSV file at any time. Airtable, Notion and webhooks are also included on every plan.

Can I stop sign-ins when the session ends?

Yes. Close the form from its settings when the session is over, so nobody adds their name later, and reopen it for the next one. Settings also include a response limit, which counts every completed response.

Why not make an attendance sheet in Google Forms?

You can, and Google Forms is free. trulyform suits a sign-in sheet that should look like your school, church or company, with your own colors and fonts, and it sends sign-ins to Google Sheets, Airtable, Notion or a webhook on every plan. There is no automatic Google Forms import yet, so start from this template instead.

Can I remove the trulyform logo from a template?

Not on Free or Pro. Business adds your logo and removes our badge. Colors and fonts are yours on every plan.
