Inside the template · 14 questions

What’s in this wedding photography questionnaire?

Start with names, an email and the wedding date, then where each of them is getting ready, the ceremony and reception venues and when the ceremony starts. A first look follow-up appears only for couples who are planning one.

Couples choose the moments on their schedule, list the family photos they want, name someone who can gather family and share anything sensitive. They can add their vendor team and a link to photos they love.

Edit the wording, add or remove questions, and change the look in the editor.

What arrives

The whole day, in one place.

The timeline, the locations and the family photo list arrive together, ready to become your shot list and photo timeline.

No more piecing the day together from an email thread the week before. Responses stay in trulyform, and you can export them as a CSV file or send them to Google Sheets or Notion.

When to send it

After booking, a few weeks before the day.

Share the link in your welcome email, and ask couples to finish it about a month before the wedding, once the timeline and venues are settled.

Their answers help you plan travel between locations, the family photos and the timing of each key moment, before you meet for a final call.

Adapt it to how you work.

  • Portraits: ask when and where the couple would like their portraits taken.
  • Second shooter: ask where your second shooter should be while the couple gets ready.
  • Inspiration photos: change the last question to a file upload so couples can add a few photos they love.

Optional questions can stay blank, and you can follow up on anything that needs a conversation.

From booking to a well-planned day.

What should a wedding photography questionnaire include?

Include contact details and the date, where the couple is getting ready, the ceremony and reception venues with start times, any first look plans, the key moments on the schedule, the family photo list, someone who can gather family and anything sensitive you should know.

When should I send the questionnaire to couples?

After they book, usually four to six weeks before the wedding, when the timeline and venues are settled. That leaves time to plan the day and ask follow-up questions.

Is this wedding questionnaire template free?

Yes. You can edit and preview the template without an account. Create a free trulyform account to publish it and collect responses within the Free plan allowance.

Can I use it as a photography client questionnaire for other sessions?

Yes. Remove the wedding questions and keep the parts about locations, people and must-have shots. It works for engagement, family or branding sessions as well.

How will I know when a couple has finished it?

Your organization’s owner gets an email as soon as the first completed response arrives. After that, new completed responses are grouped into a summary email, sent at most once an hour. Every completed questionnaire stays in your response inbox.

What is the difference between a wedding questionnaire and an inquiry form?

An inquiry form comes before booking and tells you whether the date, place and budget fit. The questionnaire comes after booking and plans the day itself.
