What’s in this wedding inquiry form?
Ask for names and an email, what the couple is planning, the date and the place. Weddings get questions about hours of coverage, guest count and an engagement session. Elopements describe their day, and engagement sessions name a location.
Everyone shares a budget, how they found you and what matters most in their photos. Only the questions that fit their plans appear, so the form stays short on a phone.
Edit the wording, add or remove questions, and change the look in the editor.

What arrives
Every inquiry arrives as a brief.
Instead of “Are you available, and how much?”, you get the date, the venue, the coverage they want and a budget range together.
Check your calendar, pick the collection that fits and reply with something useful the first time. Responses stay in trulyform, and you can send them on to Google Sheets, Notion or Slack.
Where couples find you
One link for your bio, your website and your replies.
Put the published link in your Instagram bio, behind the Contact button on your website, or in a quick reply to a DM. Couples answer one question at a time on their phone.
You can also embed the form on a page of your website, so couples never leave your portfolio.
Make the questions yours.
- Your collections: rename the coverage options after your packages, or ask about albums and a second shooter.
- Elopements: add questions about the location, permits or how many guests are coming.
- Your next step: ask whether they would like a video call or a studio visit before booking.
The form collects the details. You check availability and send your pricing yourself.
From inquiry to a useful first reply.

What should a wedding photography inquiry form ask?
Start with the couple’s names, an email, the date, the venue or city, the kind of event and a budget range. Hours of coverage and guest count help you suggest the right collection before the first call.
Is this wedding inquiry form template free?
Yes. You can edit and preview the template without an account. Create a free trulyform account to publish it and collect responses within the Free plan allowance.
Can couples fill it out on their phone?
Yes. Each question appears on its own screen and fits a phone, so it works well from a link in your Instagram bio or a reply to a message.
How do I add the form to my website?
When you publish, a “Share your form” window opens. Copy the public link for a Contact button, or paste the code under “Embed on your website” into an HTML or embed block on your page.
How will I know when an inquiry comes in?
Your organization’s owner gets an email as soon as the first completed response arrives. After that, new completed responses are grouped into a summary email, sent at most once an hour. Every completed inquiry stays in your response inbox.
Does this replace my studio CRM?
No. It collects the inquiry and keeps the answers in trulyform. If you use a CRM for proposals, contracts and payments, keep it for those steps.
What is the difference between an inquiry form and a wedding questionnaire?
An inquiry form comes first: it tells you whether the date, place and budget fit before you talk. A wedding questionnaire comes after booking and plans the day itself, such as the timeline and the family photo list.
