Inside the template · 7 questions

What’s in this RSVP form template?

Guests give their name and answer “Will you be there?” with yes or no. Both answers are required, so every reply tells you who it is from and whether they are coming.

Guests who say yes are then asked how many people are coming, including themselves, who is coming with them and about any dietary restrictions or allergies. Guests who can’t make it skip those three. Everyone can leave a phone number for last-minute updates and a message for the hosts; both are optional.

For celebrations with guests

Weddings, birthdays and parties with a guest list

It suits celebrations where you need a headcount and a meal plan: a wedding, a milestone birthday, a kids’ party, a baby shower, an engagement party or a graduation. One link replaces chasing replies across texts, calls and group chats.

Each reply lands in your trulyform inbox with the guest’s name, their yes or no, how many people they are bringing and what they can’t eat. Send the replies to Google Sheets and add up the guest counts yourself before you talk to the caterer.

Ways to adapt the RSVP questions

  • Menu choice: for a seated dinner, add a multiple choice question with the main courses, and use logic to show it only to guests who say yes.
  • Ceremony and reception: if some guests are invited to only part of the day, ask which parts they will attend.
  • Kids’ parties: change “Who is coming with you?” to ask for each child’s name and age, and ask whether a grown-up is staying.

Make these changes in the editor. The seven starter questions are listed above.

What should an RSVP form ask?

A name and a clear yes or no come first. For a wedding or a party with food, ask the people who are coming how many are in their group, who they are bringing and about dietary needs. Keep everything else optional.

Is this RSVP form template free?

Yes. You can adjust it without an account. Create a free trulyform account to publish it and collect replies within the Free plan’s monthly allowance.

How do I send the RSVP form to my guests?

When you publish, a “Share your form” window opens. Copy the public link for your invitation, a text message or a group chat, or paste the code under “Embed on your website” into an HTML or embed block on your wedding or event page. You can reopen the window from the form’s settings.

Does trulyform send invitations or reminders to guests?

No. trulyform collects the replies but does not email or text your guests. Send the invitations and any reminders yourself. The optional phone question gives you a number to reach guests with last-minute updates.

How do I count how many guests are coming?

Connect Google Sheets to add each completed reply as a row, then add up the “How many people are coming” column for the guests who said yes. You can also export a CSV file at any time. Airtable, Notion and webhooks are included on every plan.

Can I close RSVPs after the deadline?

Yes. Close the form from its settings at any time, and reopen it if plans change. Settings also include a response limit. It counts every completed reply, including guests who can’t come, so it is not a count of people attending.

What is the difference between an RSVP form and an event registration form?

An RSVP form is for celebrations with invited guests: it asks who is coming along and what everyone can eat. An event registration form is for workshops and meetups: it asks each person for an email address and accepts a maybe.

Can I remove the trulyform logo from a template?

Not on Free or Pro. Business adds your logo and removes our badge. Colors and fonts are yours on every plan.
