Inside the template · 7 questions

What’s in this quiz template?

Five general knowledge questions, each with four options. The right option is worth 1 point and the others are worth 0. Every quiz question needs an answer, so no one can skip ahead.

At the end, people give their name and, if they like, an email address. The ending screen then shows their result, such as “You got 4 out of 5 right”.

Edit the wording, add or remove questions, and change the look in the editor.

A quick check, not an exam

Where a scored quiz helps

Use it to check understanding after a lesson, a training session or a new starter’s first week, or for a bit of fun at a team meeting. It takes a minute or two to answer, and people see how they did straight away.

trulyform adds up the points for you. Each response arrives in your inbox with its score, so you can see who took part and how they did without marking a single paper.

Ways to adapt the quiz

  • Write your own questions: replace the samples with your own material. In the Score calculation, give the right option 1 point and the others 0.
  • Change the length: add each new question to the Score calculation, then update “out of 5” in the ending message to match.
  • Compare groups: add a single choice question for the class, team or session, so you can sort scores by group in Google Sheets.

Make these changes in the editor. The seven starter questions are listed above.

How do I make a scored online quiz?

Start from this template. Each question is multiple choice, and the right option is worth 1 point. A calculation adds up the points and shows the total on the ending screen, so all you need to do is write your questions and mark the right answers.

Is this quiz template free?

Yes. You can edit it without an account. Create a free trulyform account to publish the quiz and collect responses within the Free plan’s monthly allowance.

Do people see which questions they got wrong?

No. The ending screen shows their total, such as 4 out of 5, but not which questions they missed. To go over the answers together, open each response in your inbox to see the options a person chose.

Can I make a quiz like this in Google Forms?

Yes. Google Forms has a quiz mode where you set an answer key for each question. This template starts with the scoring already in place, so you only change the questions and the right answers. Responses can go to Google Sheets here too.

How do I share the quiz or add it to my website?

When you publish, a “Share your form” window opens. Copy the public link for an email or a class chat, or paste the code under “Embed on your website” into an HTML or embed block on a page. You can reopen the window from the form’s settings.

Where do I see everyone’s scores?

Completed responses arrive in your trulyform inbox with each person’s answers and score. Connect Google Sheets to add each completed response as a row, with the score in its own column. Airtable, Notion and webhooks are also included on every plan.

Can I remove the trulyform logo from a template?

Not on Free or Pro. Business adds your logo and removes our badge. Colors and fonts are yours on every plan.
