When to ask a client for a testimonial
The best moment is soon after a good result, while it is still fresh. A short form is easier to answer than a blank email, and a prompt helps people be specific.

How do I ask a client for a testimonial?
Ask soon after a project ends well. Keep the request short and personal, and say where you hope to use their words. One clear prompt makes it easier to say yes.
Is this testimonial request form template free?

How does the permission question work?
It is a required single choice with three options: “Yes, with my name and business”, “Yes, but please keep it anonymous” and “No, this is private feedback”. Each person’s choice appears with their answers in your inbox and CSV export.

How do I add this form to my website?
When you publish, a “Share your form” window opens. Copy the public link for a thank-you email, or paste the code under “Embed on your website” into an HTML or embed block on a page. You can reopen the window from the form’s settings.
Can I keep testimonials in a spreadsheet or in Notion?
Yes. Connect Google Sheets to add each completed response as a row, or Notion to add it as a database page. Airtable, Slack, HubSpot and webhooks are included on every plan too.
