Free MCP server

Create a form with Claude or ChatGPT

Connect once over MCP. Ask in plain words. Everything lands as a draft — you publish.

Updated . Other assistants, including Cursor and Claude Code, are on the MCP page.

How to create the form

  1. 01

    What you need

You need a verified trulyform account. Free is enough.

    • Claude on any plan, including Free. A Free Claude account can add one custom connector.
    • ChatGPT on the web, with Plus, Pro, Business, Enterprise, or Edu. Developer mode isn’t in the mobile apps. On Plus and Pro, OpenAI’s pages disagree about tools that create or change forms, so those plans may only read them.
  2. 02

    Connect Claude or ChatGPT

Add trulyform to Claude or ChatGPT, then sign in to trulyform to approve access. No API key or local server is needed.

  3. 03

    Ask for a draft

Describe the form in plain words, or paste one of the prompts below. Tell the assistant to save a draft and not to publish.

    Client intake for a [role]

    Try this prompt

Create a client intake form for a [role]. Ask for their name, email, what they need, their timeline, and their budget. Save it as a draft. Do not publish it.

    Event registration

    Try this prompt

Create an event registration form. Ask for the guest’s name, email, whether they can come, how many people are coming, and any access needs. Save it as a draft. Do not publish it.

    Improve wording

    Try this prompt

Improve the wording of my draft. Split any question that asks two things at once. Don’t publish.

Replace [role] with the work you do, such as a designer, accountant, or photographer. Ask for a draft. Publishing comes later, after you have read the form.

  4. 04

Review in trulyform, then publish when you say so

Open the draft and check the questions, logic, and design. Ask the assistant to publish, or publish it yourself. Nothing goes live until you publish.

  5. 05

    Make sense of the answers

Ask your assistant to summarize what people said, including how many responses it read. Free includes completed responses. Pro and Business also include partial responses.

    Try this prompt

Summarize the completed responses. Tell me how many you read, the main themes, and one thing to improve. Keep what people said separate from your suggestions.

On Pro or Business, you can ask the assistant to include partial responses as well.

Good questions

A few things you might wonder.

Still curious? Send us a note or email support@trulyform.com.

Setup guides for every assistant.

Can it publish without asking?

Yes, if you ask it to. Review the draft first. Saving a draft and publishing are separate, and trulyform does not show a new approval for every publish.

Can it delete my forms?

No. There are no deletion tools for forms or responses. The assistant can still change a draft while editing, so review those changes before you publish.

Which other assistants can I connect?

Claude, ChatGPT, Claude Code, Cursor, Codex, VS Code with GitHub Copilot, and Mistral Vibe. Each one has a setup guide.

Your draft is waiting.

Connect the assistant you already use, or open a template and edit it yourself. You publish when it’s ready.
